This exit list contains both the new and old exit numbers after MassDOT converted Route 128 to the Geographic Reference or Milepost Exit Numbering system in February 2021. MassDOT started converting exit numbers in October 2020 starting with routes in the southeastern part of the state. This project was first planned for 2016, but eventually postponed.

*Numbers in ( ) listed for the postponed 2016 Renumbering Project. These highlight the frequent discrepancy among the exit numbers and the mileposts under the new exit renumbering contract. This was done to reduce the number of lettered exits along the route, but makes some of the exits with numbers 2 miles different from the actual mileage.

1. MA 128 officially starts in Canton where I-95 and I-93/US 1 meet. MassDOT has tried with little success over the years to officially truncate MA 128 at Peabody. With this in mind, prior to 2010, mile markers along this portion of 128 started in Peabody and increased toward Cape Ann. When the route underwent a sign replacement project in 2010, the mile markers were replaced with miles indicated above with a start point in Canton. These were the mile posts used for the exit renumbering project. MassDOT officials asked why they did not use this opportunity to truncate 128 to Peabody said given the need to change other exits signs along I-95 that this action was outside the scope of the renumbering project.
